Application Forms
Admission Considerations
Admission is open to adults with all types of aphasia and all levels of impairment. In determining candidacy, previous therapy information as well as personal program goals are taken into consideration. Individuals should:

· be medically stable
· be cognitively & physically able to endure the intensity of the program
· demonstrate positive motivation for improving communication strategies
· be able to attend with a designated communication partner

Partner participation is considered an essential component of our program. However, we recognize that this time commitment may be difficult for some people. Modifications to the full partner program may be addressed on an individual basis.


Admission Procedures
Please contact us to receive an application package with the following 4 forms or click below to download a pdf version.

To be completed by your physician.

3. InteRACT Personal Program Goals

4. InteRACT Personal Program Goals for Partners

In addition to the completed forms we require a Communication Report from your Speech-Language Pathologist - (if you are currently being seen for therapy, include the report describing your current communication status. If you are no longer receiving therapy, the latest report is required).

If downloading and printing the forms, please ensure all 5 forms are completed. Attach a copy of your current communication report. Mail to:

When all of the forms have been received, we will review your file and determine whether you are a candidate for the Program.

If you are considered to be an appropriate candidate for the Program, we will send you an acceptance letter and invitation to participate. Available spaces are filled on a first-come, first-served basis in response to the invitation.

Once you have confirmed your attendance, you will receive additional forms to be completed about your communication skills. It is important that we have as much information as possible about you and your communication, in order that we may begin to plan for your Program.
